• List up to five pet names and if you want a small paw print or large paw print next to each

• Last name for the center of the disc

• Up to two birthstones for the top of the disc

• If you want all lowercase, all uppercase, or only the first letter capitalized on the names
• Any other notes or details you want to leave us :)
